If you’re looking for a USCIS I-639 physical, you’re in the right place.

How it works:

1. Sign up for an appointment through the link at the bottom of this page. This will collect your contact info and a card for payment.

2. We’ll contact you to schedule an appointment. (Please do not leave multiple messages on the office phones as this slows our ability to respond to and schedule patients).

   Please bring an interpreter with you to your visit if necessary!

   Print out and bring the most recent version of Form I-693, Report of Medical Examination and Vaccination Record to your appointment. Complete Part 1 of Form I-693 but do not sign until Dr. McCarthy instructs you to do so. We’ll use this form to document your exam.

   Bring your vaccine record and any pertinent medical history records.

3. Your visit will include the actual physical exam, screening for communicable diseases and a review of your vaccines. At the end of your visit you’ll be given an order for the required labs (these ARE included in the cost of the exam!) and guidance regarding any vaccines you might need or specialist you will need to see.

4. You’ll get your lab work drawn through our partner lab in Christiansburg, VA. As soon as results are available (typically less than 1 week) Dr. McCarthy will contact you; if all is well you can come by the office to pick up your form in a sealed envelope. If further care is needed we’ll talk you through the steps that need to be completed before Dr. McCarthy can sign the form.

5. Total cost is $400 (including the lab work)

6. Vaccines are offered through a partner pharmacy at discounted prices.

7. Once we get you scheduled we can usually get everything done within one week!
